What should I bring to my first visit?
To make your first appointment smooth, please arrive 15 minutes early so you have time to complete any additional paperwork or updates. Also, please bring:
- A valid photo ID (driver’s license, state ID, or school ID)
- Insurance card(s) if you have coverage
- List of medications you take, including dosages
- Any relevant medical or dental records, if available
